Private Duty Nursing (for adults)
Specialized health care providers serve patients who require medical care but want to maintain their independence. A private duty nurse will take the time to learn about the patient’s habits, preferences, and requirements, to tailor services to their specific needs.
Hospice
Hospice services are designed to meet the physical, spiritual, and emotional needs of clients and their families — allowing them to face end-of-life with dignity and compassion.
Pharmacy
Patients who use Phoenix Pharmacy services have a team of pharmacy professionals managing and monitoring the refill process and medical regimen. The staff works closely with each physician involved to ensure proper coordination of care.
Additionally, Phoenix Pharmacy offers a medication delivery system known as WellPack to help individuals navigate their daily medications.
In-Home Care
In-home care allows caregivers to help individuals with:
- Personal care (grooming, including bathing and dressing)
- Transportation
- Light housekeeping
- Grocery shopping
- Preparing meals
- Medication management
- Respite and advanced respite care
To qualify for in-home services, the individual must be elderly or disabled, eligible for Medicaid, and assessed by the Division of Senior & Disability Services.
Comprehensive Disease Management
Comprehensive disease management, or CDM, is a holistic approach to managing chronic and life-limiting diseases. The CDM program was developed to assist people in optimizing the utilization of all available resources to establish goals of treatment and advance care planning.
